Actually it's telling the story about since David's girlfriend died he's been having feelings of killing himself and joining her "Inside the Fire". Or something like that.

She's committed suicide, and he just has the voice of the devil over his shoulder trying to provoke him to take his own life to join her again, because the only chance for him to be with her again is if he takes his life. Edit: IMO, it was probably best to make this song and let that out then keep in bottled in and whatnot. Or else we'd lose the singer of one of the best bands ever.
